Thank you for your patronage of Valley Vet Supply and ValleyVet.com.  In order to consider your request, it will be necessary for you to submit on your organization's letterhead the following information:

1. name of organization
2. UPS deliverable address of the organization
3. telephone no. of the organization
4. deadline date of arrival of donation
5. contact person's name & daytime telephone no.
6. description of the event, i.e. barrel racing, breed show, etc.
